Selecting the proper utility service applications for your project
A Customer Service Information Sheet [Form 4411, 11/08] must be submitted with all utility service applications required for your new project. The CSIS provides our representative with project contact and ownership information The CSIS also serves as a checklist and description of the various applications so you can determine what applications are needed.
For example, the CSIS checklist would specify these required documents for a residential plat development:
A multifamily development project requiring both natural gas and electric service would identify these required documents:
A mixed use project is a combination of non-residential, plat, and residential multifamily units in the same project
(e.g. high rise multifamily apartments/condominiums with retail on street level)
- A combination of applications from residential and non-residential will be required.
