The Customer Service Information Sheet [Form #4411, 11/08] gathers project information and ownership details. It also includes a checklist to help you verify the necessary applications to be submitted to PSE.
Non-residential temporary electric service - 200E
Use this application for non-residential temporary electric service installed to provide power for a period of 12 months or less. This application should be used for multifamily buildings and non-residential new construction projects that require single or 3-phase service for construction trailers, tools, cranes, lights, elevators, etc.

Non-residential permanent electric service - 201E
Use this application for all new buildings whether the whole structure or a part of it is intended for commercial, retail, or general public activities requiring natural gas and electric service. This application also should be used for electric service to residential community wells, barns and garage shops/outbuildings.

Non-residential gas service - 201G
Use application 201G for existing non-residential structures converting to natural gas from an alternate fuel, e.g. oil, propane, or electricity.

Multifamily development electric and gas service - 400E and 400G
Use these applications to obtain electric and/or natural gas service for two or more dwelling units within a single structure, e.g. duplexes, triplexes, four-plexes, and apartment/condominium complexes. This may include one or more structures.
