I certify that I am the property manager and/or the landlord and that I am in possession of the Puget Sound Energy Start Service Authorization Form. This form has been signed by the tenant and has been or will be uploaded as a step of the Landlord/Tenant start service process.

Furthermore, I agree that if I am unable to produce this signed Puget Sound Energy Start Service Authorization Form in the event of a dispute related to the tenant start service date, I will pay the charges from when the service started up to the date that the tenant agrees to pay.
